About you:

You'll:

• Deliver IV therapies, subcutaneous injections, and venepuncture in line with patient care plans
• Conduct holistic clinical assessments to tailor care to individual needs
• Collaborate with patients, families, and healthcare professionals to support recovery
• Maintain accurate records and uphold professional nursing standards
• Travel across the region to provide care in patients' homes (full UK driving licence required)
• Champion patient-centred care and continuous improvement
